This edition of the Combined Membership List (CML) includes the names and addresses of all persons who were members of AMS, MA A, or SIAM as of June 1,1994. The information in this book is taken from membership records retained in the AMS computer. It was assembled using programs created by the Society for this purpose. This directory is distributed in even-numbered years to AMS members on request to the AMS and in odd-numbered years to MAA members on request to MAA. The CML is divided into three sections: an alphabetic list of individual members; a geographic list of individual members; and a list of academic, institutional, and corporate members. Abbreviations used in the lists are given on the pages following this foreword.
Alphabetic List. The first section is an alphabetic list of individual members. Each entry includes a name, mailing address, society affiliation(s), and, if supplied, title or position, place of employment, temporary place of employment, a telephone number, and electronic addresses. When provided, accents are included in names. Membership in the three participating societies is indicated by the following letters, placed at the end of each entry:

The mailing address follows the name of the individual and is preceded and followed by asterisks. Where no department is specified with academic addresses, readers should assume it is the department of mathematics. State and territory names are abbreviated according to U.S. postal regulations and are followed by ZIP codes.
Geographic List. The second section is organized geographically into three subdivisions: the United States, Canadian provinces, and other countries. Names of members are arranged according to city and state; two or more members employed at the same institution are listed alphabetically after the employer's name. ZIP codes immediately follow the names of employers in the United States. Members who have submitted notification of leave of absence are listed under the institution which has granted the leave.
Academic, Institutional, and Corporate Members. The third section is a list of academic, institutional, and corporate members of the participating societies. Telephone numbers of university switchboards and departments in the mathematical sciences, as well as electronic addresses, if provided, have been included.
